Iron Lung places us in the role of a prisoner forced to carry out a suicide mission. The game’s world is set after a mysterious event known as The Quiet Rapture, in which nearly all of humanity—and even the stars in the universe—vanished without explanation. The remaining humans now rely on exploring alien moons in order to survive.
We are sent to a moon filled with an ocean of blood, using a small and fragile submarine called the Iron Lung. There are no companions, no rescue, and almost no hope of returning. The story is not told through long cutscenes, but instead through documents, maps, and photographs we take ourselves. The result is a narrative that feels cold, silent, and full of mystery.
In terms of gameplay, Iron Lung is far from complex. Throughout the entire game, we never leave the submarine. All activities are carried out within a cramped room filled with buttons, levers, and navigation instruments.
We must read maps, match X and Y coordinates, and slowly maneuver the submarine toward the target location. Once there, we take photographs using a camera that produces blurry, noisy images. There are no enemies to fight, no weapons, and no upgrade systems. Even a small mistake can be fatal.
Sounds boring? Quite the opposite. Because visual information is extremely limited, every decision feels tense. We never truly know what lies outside the submarine, and that uncertainty constantly weighs on the player’s mind.
Iron Lung is a perfect example of psychological horror. The game does not rely on cheap jump scares. Fear is built through a claustrophobic atmosphere, a sense of isolation, and strange sounds coming from outside the submarine’s hull.
The absence of background music makes the sound of the engine, metallic creaks, and mysterious noises feel far more disturbing. Even silence becomes terrifying. We are left constantly wondering: is there really something out there… or is it just our imagination?
The level design in Iron Lung is limited, but intentionally so. The game map is filled with narrow passages that look almost identical to one another. Without maps and coordinates, getting lost is almost guaranteed.
Interestingly, this confusion is not a flaw, but a core part of the horror design. Players are forced to stop, think, calculate distances, and carefully confirm directions before moving. All of this reinforces the feeling of isolation and psychological pressure.
Visually, Iron Lung is very simple, using a low-fidelity black-and-white style. The graphics are rough, grainy, and far from realistic. However, this actually causes the player’s imagination to work harder, filling in the visual gaps with fear.
Meanwhile, the audio is the true star of the game. The creaking submarine, rising pressure, and strange sounds from outside create an intensely immersive atmosphere. There are many moments where players will hold their breath—just because of a small, inexplicable sound.
